and i have requested a passport application form from the (Embassy of Ireland) because thats where jamaica embassy of ireland is located and i havent recieved the passport application form via the canada postal mail from last month.

A form sent from Ireland will not help since the passport application form used in the Republic of Ireland (APS1) is different from the form used for an overseas application (APS2E/G). The address you have used is correct except that you should have entered "Passport Officer" in the first line. Maybe your letter was missed somewhere in the distribution system of the Embassy. If you do not find anybody living near to the Irish Embassy or a Honorary Consulate in Canada I recommend that you send them an e-mail and request another passport application form to be mailed.

part of your problem is your trying to get something from ireland through a country that is thousands of km away from ireland. if you get something from ireland you won't be getting it through canada post unless you live in canada
